At Total Care Connections, we're passionate about helping seniors and individuals remain safe, independent, and cared for in the comfort of their own homes. We're looking for a compassionate, organized Care Coordinator to join our growing team and play a vital role in delivering exceptional care.
As a Care Coordinator, you'll be the bridge between clients, families, caregivers, and our internal teams—ensuring every client receives personalized, high-quality care while supporting our caregivers every step of the way.
What You'll Do
- Conduct in-home client assessments and supervisory visits
- Develop and update individualized care plans
- Build strong relationships with clients, families, caregivers, and referral partners
- Support caregiver onboarding, training, and ongoing supervision
- Ensure compliance with company policies and Medicaid regulations
- Maintain accurate documentation and participate in quality improvement initiatives
- Collaborate with Scheduling, Recruiting, and Operations to deliver outstanding service
What We're Looking For
- At least 1 year of experience in care coordination, care management, or care supervision
- Strong customer service, communication, and relationship-building skills
- Excellent organization and attention to detail
- Ability to manage multiple priorities in a fast-paced environment
- Reliable transportation and a clean driving record
