Intake Coordinator

Job Summary
The Intake Coordinator plays a vital role in growing our client base by cultivating strong referral relationships, conducting timely client assessments, and building individualized care plans. This is a Monday–Friday, 8:00 a.m.–5:00 p.m. in-person position based in Colorado Springs, CO.
Success in this role requires self-motivation, consistent follow-through, and the ability to meet clearly defined performance targets.
Duties and Responsibilities
Referral Partner Engagement
- Develop and maintain strong relationships with the Colorado VA and The Resource Exchange (TRE), as well as other community referral sources.
- Conduct at least 50 outbound calls per day to referral sources and/or prospective clients.
- Send follow-up emails for every referral and a thank-you card to every referral partner.
- Track and meet the following weekly goals:
- 50 new TRE hours per week
- 25 new VA hours per week
Client Intake and Care Planning
- Conduct in-home or on-site new client assessments and meet with families to identify needs.
- Create comprehensive, individualized care plans that align with assessment findings.
- Ensure care plans are clear, goal-oriented, and initiated promptly after intake.
- Communicate care plans to caregivers and operations team members for smooth service initiation.
Compliance and Documentation
- Ensure all intakes and care plans meet Colorado Medicaid regulations and company standards.
- Maintain accurate, timely documentation in the agency’s systems.
- Understand and apply eligibility requirements for HCBS, VA programs, and TRE services.
Collaboration and Communication
- Partner with Care Managers and Scheduling teams to ensure new clients are staffed quickly.
- Represent Total Care Connections professionally when meeting with clients, families, and referral partners.
- Participate in team meetings and contribute to growth-focused initiatives.

Skills/Qualifications
- Preferred: Experience working with Colorado VA and/or TRE referral processes.
- Preferred: In-depth knowledge of Colorado Medicaid HCBS regulations.
- At least one year of experience in the home and community-based services industry in Colorado.
- Proven ability to generate and grow client referrals from professional partners.
- Excellent communication and relationship-building skills.
- Strong organizational skills with attention to detail.
- Ability to work independently and meet high daily/weekly activity targets.
- Proficiency with computers, CRM systems, and email communication.
- Reliable vehicle and clean driving record.
