Are you a dynamic leader with a passion for delivering high-quality healthcare services? Do you have proven experience in launching and growing a Medicare-certified home health division? If so, Total Care Home Health, a newly established division of Total Care Connections, is looking for you to lead our exciting venture.

As the Director of Total Care Home Health, you will spearhead the launch of this new division, ensuring compliance with Medicare, state, and federal regulations while driving its growth across multiple locations. You will work closely with the CEO and leadership team to set strategic goals, build a high-performing team, and develop lasting partnerships with key stakeholders, including the VA, Department of Developmental Disabilities, and more.

Key Responsibilities:

• Lead the startup and ongoing operations of Total Care Home Health, including policy implementation, contracting, and compliance.
• Oversee EHR implementation and ensure continuous survey readiness for ACHC accreditation.
• Build and manage a high-performing team, including clinical staff and leadership positions.
• Establish and maintain contracts with key partners, such as TriWest and the Department of Developmental Disabilities.
• Collaborate with the CEO and leadership team to drive growth, expand into new locations, and ensure financial performance.
• Develop and implement quality improvement programs focused on patient outcomes and satisfaction.
• Work closely with the business development, HR, and scheduling teams to support overall company goals and objectives.
• Prepare the division for the upcoming ACHC survey by February 2025.

Qualifications:

• Proven leadership experience in a Medicare-certified home health setting.
• Strong understanding of Arizona Department of Health regulations and Medicare compliance.
• Exceptional communication and organizational skills.
• Ability to lead and inspire a team, and to collaborate effectively with various departments.
• Experience in business development, contract negotiation, and start-up operations.
• Familiarity with EOS (Entrepreneurial Operating System) is a plus.

Education and Experience:

• Bachelor's degree in Healthcare Administration, Nursing, or a related field (Master's preferred).
• Minimum of 5 years in a leadership role within a home health or healthcare organization.
• Experience with home health operations and start-up environments is highly desirable.
