The Senior Home Care Specialist represents Total Care Connections in the local community, creating and nurturing relationships with healthcare professionals and organizations. This key liaison ensures Total Care Connections is the primary choice for compassionate, high-quality senior care. From initiating contact to maintaining enduring relationships with referral sources, this role is fundamental in promoting and upholding the company’s reputation.
- Advocate for the company’s mission and services within the local community.
- Establish and fortify relationships with case managers, discharge planners, and other pivotal referral sources.
Target Account Management:
- Cultivate relationships with 40-50 target accounts, aiming to connect with at least 3 referral contacts within each entity.
- Regularly visit 8-10 accounts daily, building rapport and showcasing how Total Care Connections can meet their needs.
- Spearhead marketing events such as "lunch and learns" and networking gatherings to bolster the company’s presence.
- Proactively participate in community events, seminars, and workshops pertinent to senior care.
- Diligently follow up with all referral sources, offering updates on care plans and services once clients are onboarded.
- Serve as the primary contact for all referral inquiries, maintaining the company’s reputation for reliability and excellence.
Continuous Learning and Advocacy:
- Stay updated on industry trends, challenges, and needs, adjusting strategies accordingly.
- Guide families and potential clients by providing relevant insights, recommendations, and advice.
